For months, I’ve terrorized my Dungeons & Dragons players with an unusual monster – the aboleth, a large three-eyed fish with minor psionic abilities whose existence pre-dates the gods themselves.

Horror is hard to do in a tabletop game ... beholder named Xazax in Out of the Abyss (2015). The multitudinous aboleth saw the beginning of time itself, before the gods, before this world ...

Perhaps a bit of a traditional option, but you cannot deny the horror of the Aboleth. A serpent-like creature whose face is mostly mouth with several roles of sharp teeth, ready to rip its prey apart.
